What this form is for
This is the patient inquiry path for LiveWell Psychiatry and Men's Health. Use it if you're looking for psychiatric care, medication management, ADHD evaluation, anxiety or mood care, sleep help, men's health care, sexual health care, hormone-related questions, or a practical conversation about whether LiveWell is the right place to start. It's also the right form if you're a parent, partner, or family member trying to help someone get connected.
The form isn't a crisis line, patient portal, prescription refill line, or way to change an existing treatment plan. If there's immediate danger, call 988, call emergency services, or go to an ER. If you're already a LiveWell patient, use the channel your care team gave you. If you're writing about a website correction, citation, press request, or article question, use the editorial contact page instead.
Where we can see patients
LiveWell clinicians are licensed for care in Oregon and Washington. If you live outside those states, you can still submit the form, but the team may need to point you toward a different option. State rules matter in telehealth, medication prescribing, and follow-up care, so location is one of the first things the intake team checks.
What helps intake move faster
The most useful inquiry says what you're hoping to treat, what you've already tried, what medications you're currently taking, how soon you want to be seen, and whether insurance or cash pay matters. Skip the whole history here. Enough context to route the request beats a long message full of details you'd rather discuss with the clinician.
Insurance and fit
The care coordination team checks insurance, specialty fit, age, state, and the type of help you're looking for. Sometimes that means scheduling. Sometimes it means explaining why a different level of care, different specialty, or different clinic would be safer. A good referral isn't just a fast appointment. It's the right kind of appointment.
What happens after you send it
A coordinator reviews the inquiry and usually responds within one business day. They may ask a few practical questions before scheduling. If the request looks urgent, outside LiveWell's scope, or better handled somewhere else, they'll say that directly and try to point you toward a better next step.
